LifeScape to host open house events for Conflict-Free Case Management services

The first is Thursday, April 27, 11:30 a.m.-1:30 p.m.  The second is Monday, May 1, 4:00-6:00 p.m.  Both will be held at the LifeScape Conference Center, 4100 S. Western Avenue in Sioux Falls.

LifeScape offers case management to South Dakota residents with intellectual/developmental disabilities, who receive services through the HCBS Choices Waiver in the Southeast Region. This includes the following counties: Minnehaha, Lincoln, Yankton, Union, Clay, Turner, Hutchinson, Bon Homme, McCook, and Hanson.

LifeScape’s three case managers are Nate Alfson, Lyndsey Ihler, and Nathan Jones. They are trained in person-centered planning and focus on personal outcomes for the people they support. They assist people in developing their Individual Support Plan (ISP), monitor their plan, and help them in self-directing their supports and services.  Once LifeScape is selected for a person’s case management, our staff will visit to set up services, make quarterly on-site observations, and meet for yearly review/revision of the person-centered plan. 

LifeScape was the first agency in the United States to achieve accreditation through the Council on Quality & Leadership (CQL). It has maintained this accreditation for 37 years, the longest of any organization in the world.
